Physical Therapist

Physical Therapist (PT) – Home Health

We are seeking a licensed Physical Therapist (PT) to provide high-quality home health services to patients throughout Forsyth, Davidson, Stokes, and Surry counties. The PT will be responsible for evaluating, planning, and implementing individualized treatment programs to help patients achieve their highest level of independence and functional mobility.

Schedule: Monday–Friday
On-Call: None
Mileage Reimbursement: $0.43 per mile
License Requirement: Active North Carolina Physical Therapist License

Key Responsibilities:

• Perform comprehensive physical therapy evaluations based on physician orders and referrals.
• Develop individualized treatment plans with measurable goals.
• Assess patient progress and modify treatment plans as needed.
• Provide therapeutic interventions to improve mobility, strength, function, and independence.
• Communicate patient progress and care needs with physicians, caregivers, and interdisciplinary team members.
• Recommend and assist with adaptive equipment and assistive devices.
• Complete accurate and timely documentation, including OASIS assessments and billing records.
• Ensure compliance with company policies, HIPAA regulations, and state and federal guidelines.
• Supervise Physical Therapist Assistants (PTAs) and therapy aides in accordance with state regulations.
• Participate in staff development, continuing education, and team meetings.

Required Skills:

• Home Health experience
• OASIS documentation
• HCHB (Homecare Homebase) experience
• Strong clinical assessment and communication skills
• Ability to work independently in a home health setting
